Our team focus on healthy lifestyle and independence whether that is physically, psychologically, cognitively or socially. Our Allied Health professionals also support the management of people with disabilities from childhood to adult.

Nurse Practitioner - Chronic Disease / Diabetes
A Nurse Practitioner provides comprehensive clinical care for clients with diabetes, cardiovascular disease, renal disease, and other related complications of diabetes.

A Physiotherapist can assess, diagnose, treat and prevent a wide range of health conditions and movement disorders. Treatment is aimed at repairing damage, reducing stiffness and associated pain, increasing mobility and quality of life improvement. Physiotherapy ranges from health promotion to injury prevention, acute care, rehabilitation, maintenance of functional mobility, chronic disease management, patient and carer education and occupational health.
