ALLIED HEALTH SERVICES Northern Australia Primary Health Limited is funded by the Northern Queensland Primary Health Network to provide Outreach Allied Health services to the areas listed below. If you would like us to provide a service to additional areas please contact us via our Individual and Group Allied Health services are provided in Townsville at minimal or no out of pocket expense to the individual.   • Diabetes Educator

    • Helps you understand what happens when you have high or low blood sugars
    • How food and exercise help you stay healthy for the future
    • Helps people who need insulin or tablets to control their diabetes

     

    This service available in:

    Ayr, Bowen, Cardwell, Charters Towers, Clermont, Collinsville, Dysart, Glenden, Home Hill, Hughenden, Ingham, Middlemount, Moranbah and Richmond

  • Dietitian

    A dietitian can help you understand how different foods play a part in your health and can provide information specific to the needs of people who experience the following:

    • Overweight or obesity
    • Malnutrition
    • Heart disease
    • Kidney disease
    • Diabetes
    • Stomach and intestinal conditions
    • Food allergies and intolerances

     

    This service available in:

    Ayr, Bowen, Cardwell, Cannonvale, Charters Towers, Collinsville, Hughenden, Ingham, Proserpine, Richmond, Sarina and Isaac Region

     

  • Exercise Physiologist

    An exercise physiologist can help people with exercise and eating for health. They also can:

    • Create exercise plans for people recovering from injury
    • Help people to understand exercise and how eating well can lower risks of developing diseases such as heart disease and diabetes in the future

     

    This service available in:

    Cardwell, Charters Towers, Hughenden, Ingham, Richmond, Townsville, Ayr, Bowen, Cannonvale and Proserpine

  • Occupational Therapist

    • Help people to live more independently at home, finding ways to make the home easier to live in
    • Help make tasks easier to complete
    • Provide access to home care and home modification equipment
    • Help children grow independent
    • Teach people ways to limit risk or falls and injury
    • Work with people who have had hand surgery to increase strength and use of their hand
    • Run exercise classes to increase strength and mobility such as wheel chair groups and Tai Chi

  • Healthy Eating Activity and Lifestyle (HEAL)

    HEAL is a lifestyle modification program that enables participants to develop lifelong healthy eating and physical activity behaviours. In it’s original form HEAL™ consists of eight weekly group education and group exercise sessions as well as individual consultations pre- and post-program and five and 12 month follow-up health consultations. Each week participants undertake one hour of supervised group-based low to moderate intensity physical activity followed by a one hour group-based healthy lifestyle education class. HEAL™ is a nationally accredited evidence based program run by Northern Australia Primary Health Limited. HEAL can be delivered in a number of formats to suit your organisation’s requirements and other services such as mental health incorporated into a package for your needs.

     

    Who can participate in HEAL™?

    The program assists people who are overweight, or obese, with existing chronic disease or at risk of developing chronic disease to improve their health and well-being by providing lifestyle education with a focus on improving nutrition and physical activity and promoting self-management. Persons at risk of developing, or with existing diabetes, heart diseases or other conditions are examples of some of the participants that can attend HEAL™.

     

    How can I refer patients or become a participant of HEAL™?

     

    Eligible patients include those:

    • at high risk of developing Type 2 Diabetes
    • with 2 or more cardiovascular disease risk factors
    • with a BMI greater than 30

    HEAL can be also offered to those without existing risk factors such as general community groups to raise awareness of health promoting practices. NAPHL has successfully run HEAL with employees, teachers, general health workers, disability support workers and Indigenous groups. A free program is available in most rural areas, while in others HEAL is offered for a fee.

     

    Referring agencies or patients are required to fill in an Outreach Allied Health Service referral form and HEAL screening tool or contact our Exercise Physiologist below to work out a package that suits you.
